Tio Pakusadewo showed early interest in the arts, taking up dance in elementary school and joining his senior high school's theatre troupe. After graduating in 1985, he enrolled at the Jakarta Art Institute while working as a model. He made his feature film debut in 1987 opposite Rano Karno in Bilur-Bilur Penyesalan. National recognition came in 1988 with roles including Catatan Si Boy II, and his fame was cemented by his role in Garin Nugroho's Love in a Slice of Bread (1990), which won the Citra Award for Best Film. In 1991 he received his first Citra Award for acting for Lagu Untuk Seruni.

During the early 1990s, as the domestic film industry declined, Tio focused on soap operas. In 1993 he established his own production house, Kalbuku, which produced several serials but soon faced financial problems and closed; he also began facing a drug addiction. His last feature film role of the millennium was in the indie film Kuldesak (1999). He did not reenter the film industry until 2004, with a role in Virgin. Subsequent notable films include Berbagi Suami (2006), Legenda Sundel Bolong (2007), Quickie Express (2007), Pintu Terlarang (2009), Sang Penari (2011), and Letters from Prague (2016), for which he won best actor awards. In 2010 he was granted the title Kanjeng Raden Tumenggung from the royal court at Sultanate of Yogyakarta and established an acting school in Kemang, South Jakarta.
